Steps to obtain NYS Medical Marijuana Certification:

• Fax/ Send medical records that pertain to your illness (noted above)

• We will call you as soon as your records are reviewed to arrange an appointment

• Our providers with meet with you, give you a certificate and register you on the NYS DOH website and show you how to register yourself on the patient side of the DOH website

• You will be mailed a medical marijuana card within a week

• Once you receive your card, you may purchase products from any NYS certified dispensary
